Active Teens Participant Registration Survey

Congratulations on taking part in the Active Teens Program. To be eligible for the Active Teens Program you must be aged 13-17 years living in the Noosa Shire. The following questions help us evaluate the program and its aims as part of our reporting requirements to our funding body. Having this evidence also helps us demonstrate demand for this type of program when future funding opportunities are available.
